BSA National Four Part Medical Form - 2011 version Now one form for all events National and Denver Area Council (no more Colorado Health Form) This one PDF contains Parts A, B, C and D. Once completed it is valid for 12 calendar months from the date of doctor's signature. This is the form for Summer Camp and High Adventure and it can be used for short term camping as well.
BSA National Medical Form Parts A & B Only For short term (weekend) events lasting less than 72 hours. This is the same Part A & B that is included with the full four part form above, so if you have completed parts A & B previously for summer camp, they are still valid for 1 year from date of signature.
Mission: To prepare young people to make ethical and moral choices over their lifetimes by instilling in them the values of the Scout Oath and Law.
